We are a Minneapolis-based, private 245D licensed provider that serves clients who need care and support in the comfort of their own residence, and some community living services. 

Our goal is to support YOU, how YOU want to be supported.

You are at the center of your support plan.

We strive to offer individualized support plans based on the needs of our clients. In doing so, we:

  • Develop and maintain a trusting relationship.
  • Provide a safe environment.
  • Collaborate with several qualified physical and occupational therapeutic agencies to provide teaching and training to patients and families.

We aim to provide care and support to our clients with utmost respect and integrity. We aim to provide assistance where and when needed, fostering independence for our clients through a person-centered approach, and providing an environment where they are empowered to lead their lives as anyone else in the community.
